October 6, 1986 — The Soviet submarine, K-219, with 16 ballistic missiles each carrying 2 warheads, sank about 600 miles east of Bermuda. One of its nuclear reactors had overheated and seaman Sergey Preminin manually shut it down, but sealed his death in the process. It was later revealed that highly radioactive plutonium 239 was released in the mishap.
